Confirmed cases

If you are advised of a confirmed COVID-19 case affecting an employee or contractor please immediately complete the COVID-19 Positive Case Report form (24 hours).

For next steps follow the Positive Employee flowchart and use the NSW Health contact risk assessment for community and workplace settings to guide you. These can all be found in the quick reference guide to reporting and communicating cases.

The following procedure gives further guidance on managing and reporting confirmed cases of Coronavirus across the Transport cluster.

COVID-19 - Managing and reporting cases procedure

What you must do 14 days after reporting a COVID-19 positive case

14 days after you have reported a Covid-19 positive case, you will receive a notification to fill out the Positive Case Update Form. Please complete the form immediately. Use the positive case update flow chart to guide you through the process.

If the case is to be closed prior to the reminder email prompt, an email can be sent to the COVID-19 Taskforce to request an update of case information.

Am I required to notify SafeWork NSW of confirmed cases of COVID-19?

If your worker has contracted COVID-19 at work, then you should notify SafeWork NSW. For further guidance and assistance with notification, please contact your Health & Safety partner.

Reporting self-isolation cases

If you have a team member who is required to self-isolate as a result of exposure to COVID-19 either at or outside of the work place, you must immediately fill out the self-isolation reporting form. The self-isolation reporting process flow chart explains what you need to do. There are several ways an employee may find out if they are close or casual contact. The NSW Government’s rules on self-isolation is available here. Use this guide to support your people - when a team member is in self- isolation.

Coughing/spitting incidents

To report a coughing or spitting incident affecting any Transport employee or contractor, please complete the Spitting and Coughing reporting Form.
